What to Bring

Please bring the following to your appointment (for all patients, including returning patients):

  • An up-to-date list of your current medications

  • Specialist letters or reports — including cardiology, respiratory, rheumatology, orthopaedics, gastroenterology, etc.

  • If you don’t have copies, please write down the names of the specialists you’ve seen

  • Imaging reports (x-ray, ultrasound, MRI)
